    entertainer booking sheet/ inquiry form

What do you use when someone calls? I am updating my (paper) from to make it more useful. Do you add the information into a database?

    Dave

PS any samples would be quite useful.

We do almost all bookings via email/web. I can't remember the last time we got a phone call. And yes, it goes into a database.

    Our contact form is here:

Most people don't fill in budget, but some do and it's useful.

      database

I have a database system designed in filemaker pro.

As I talk to people on the phone or from information provided via email or other notes, I enter it into the main contact page which then gets put into the database automatically, no need to hit a save button.

I can hit a variety of buttons from Contract, fair/festival contract, theatre contract, invoicing, mailing, shows list, calendar etc and it will input the information from that contact page into the proper places of that particular page, including pricing, additional notes, when I sent the contract and more.
