tricky auto forms on my website.

    OK here it is.
I have just more or less completed an overhaul of the performance side of my website.
    what used to be

    is now

Yeah yeah whatever, so now i spoonfeed the client rather than thinking up new pretentious terms for 'roving'

anyway i did all this so that i could move onto a project that actually interested me and I've more or less finished it but need a hand.
Now i am introducing in my 'Products' section (note: I have never in 3 years sold ANYTHING)
and also cross referencing into my services section, a new and wonderful cash generating concept.
    OUTSOURCING SELF HATRED

I would like to design some sort of form that the customer would fill in
Sex/e-mail/ list of negative personality aspects
so that i can send them a form reply once a month with as little actual input as possible.
also could someone e-mail me and talk me through a pay-pal account.
Heres to my continued spectacular and dramatic failure as a businessperson
